I would assume that's up to the GM, like it is with most tabletop games. DnD can be "quest-based", sure. I've also seen DnD campaigns that are epic, year-long things with a massive over-arcing plot. It just depends on the players and how their GM wants to run it.

I left the setting up to the GM to devise, with no actual end game to speak of. I assume you could have the PCs go through episodes or story arcs, with maybe further growth beyond Region Size number 4. Perhaps their Size Totals could hit 15 each, rendering them nearly immobile?

In any case, you don't have to do one-shot sessions.
